Your experiences of living with arthritis will help inform our work and make sure we effectively raise awareness of how people manage their life with arthritis.
Are you a parent or carer?
If you are a parent or carer for someone with arthritis, please fill out the form with respect to their condition. For example, for Date of Birth please use their date of birth. However, please use your name and contact details so that we can contact you for projects in the first instance.
For some questions, such as how arthritis affects your life, we are interested in how it affects both you and the person you care for. These questions have an additional help button next to them to give you guidance on the topics you may wish to include.
